 Read the "Copyright Agreement" very carefully and check it to indicate you agree to abide by it.
 Click the "Submit" button and your application will be e-mailed to us.
 We will then call you on the phone to verify your application and make final arrangements with you.

 After we call you may mail a check to us.
 If you desire you can apply your charges on your credit card. We will call you to ask for your preference, or you may call us at the C.L.T.I. office: (503) 255-3540.

I UNDERSTAND THAT ALL OF THE MATERIAL IN MY "LEARNING PACKET" IS COPYRIGHTED. I AGREE NOT TO DUPLICATE ANY PART OF IT FOR ANY REASON.

RETURN POLICY: If any of the material in your learning packet is damaged, please return it to the C.L.T.I. office within 30 days and we will replace it immediately.

REFUND POLICY: If your course(s) is unsatisfactory and you return it in its entirety and undamaged within 30 days of receiving it, all your charges (except the non-refundable enrollment fee) will be refunded.
